1. Shareholder and director identification
- Indonesian shareholders/directors: a valid KTP (national ID card) and personal NPWP (tax ID).
- Foreign shareholders/directors (PT PMA): a valid passport, and for resident directors, the relevant stay/work permit documents.
Names and details must match exactly across every document — small inconsistencies cause rejections.
2. Company name
A proposed company name (for a PT, usually three words) that is available and compliant for reservation with the Ministry of Law and Human Rights.
3. Business activity (KBLI) codes
